7. Complete and submit signed FERPA form

8. Complete, notarize, and submit the Barry University Hold Harmless form
□
9. Complete and submit the Holy Cross Hospital Non-Disclosure form
□
10. Complete and submit the City of Fort Lauderdale Hold Harmless form
□
