I, the undersigned aant, being ﬁrst duly sworn, hereby state:

1. This adavit is being sworn regarding the property located at ___________________________________________

(“Property”) that is/shall be used as a short-term rental.

2. As of the date of this adavit is signed:

 I am the sole owner or one of the joint owners of the Property.

 I am the property manager or agent of the owner authorized to apply for a short-term rental license and swear to

this adavit.

3. The Property shall be listed only on short-term rental marketplace websites (e.g., Airbnb) that provide the one million

dollars ($1,000,000) liability insurance policy required under Boise City Code Title 3, Chapter 22 for short-term rentals.

4. Please list the websites you are or intend to use to list the Property:

I shall obtain such insurance coverage and submit the Certiﬁcate of Liability Insurance form to the Boise City Clerk’s

Oce prior to listing on such websites.

I hereby state the above information is true, accurate, and complete, and relevant information has not been omitted. This

ends my adavit.
