Notes
This certificate is valid for 10 days from the date of issue by the official veterinarian. In the case of transport by sea,
that period of 10 days is extended by an additional period corresponding to the duration of the journey by sea. [Even
t
hough it states that the certificate is valid for 10 days, it must be signed by the USDA Accredited or Military* Veterinarian and
endorsed by APHIS (if not issued by a Military Veterinarian*) within 48 hours of the animal leaving the United States. The certificate
is then valid for 10 days after signing in case the animal travels to the EU by sea. The 10 days would not apply if traveling by air as
flights to the EU are less than 24 hours.]

(2) Any revaccination must be considered a primary vaccination if it was not carried out within the period of validity of a

and 21 days must elapse before the animal is eligible to enter the EU.]
(3) A certified copy of the identification and vaccination details of the animals concerned shall be attached to the certificate.

Treatment must be administered by a USDA Accredited Veterinarian. The treatment may be administered before or after the health certificate is
endorsed (signed and stamped) by the APHIS veterinarian, but the administering veterinarian cannot sign the table until after he/she administers
the tapeworm treatment.

moved in accordance with Article 5(4) of Regulation (EU) No 576/2013 of the European Parliament and of the
Council;
